WHAT IS HOSPICE?
Hospice is a healthcare program designed to meet the special needs of terminally ill patients and their loved ones. The program can significantly improve the quality of life for patients who are unwilling or unable to seek aggressive treatment for their condition.
WHO IS ELIGIBLE?
Patients with a confirmed diagnosis of terminal illness and a limited life expectancy. Anyone concerned with patients well being can make a referral. Common referral sources include: the attending physician, a family member, a hospital discharge planner, clergy, a social worker, a friend of the individual directly.

The terminally ill patient’s care is provided by an interdisciplinary team including:
HOW DO WE START THE PROCESS?
A single phone call can start the process. Once we receive a call from anyone of the above referral sources, we will contact the patients attending physician for approval. As soon as the approval is received the hospice process can begin.
WHO PAYS FOR HOSPICE?
Hospice is covered by Medicare, Medi-Cal, health insurance, HMOs and private payer sources.
OUR BELIEF:
Alpenglow Hospice believes that the patient is best served in familiar, comfortable, and appropriate surroundings. For this reasons, patients may receive hospice care in a variety of locations, including their home or at a care center. While a family member or other caregiver provides daily care, members of the hospice team make regularly scheduled visits to the patient. When needed, a skilled hospice nurse is available on call, 24 hours a day to provide advice over the phone or to make an emergency home visit if necessary.
